AFC Care Manager

Schedule is: Mon-Fri 8a-4p

Salary starting at $55,000.

The Care Manager is responsible for the coordination, implementation, and ongoing management of care management services, caregiver support, and qualified settings for individuals enrolled in the Adult Foster Care (AFC) Program. The Care Manager works collaboratively with the Director of Community and Family Services, AFC Registered Nurse, AFC caregivers, members, families, responsible parties, and other members of the multidisciplinary team (MDT) to ensure the delivery of quality, person-centered services in accordance with MassHealth regulations, applicable state and federal requirements, and LifeStream policies and procedures.
