Rocky Mountain Care is growing our home health services in the Rawlins and Carbon County area, and we're looking for compassionate, dependable Certified Nursing Assistants (CNAs) who want to make a meaningful difference in the lives of the people they serve. As a Home Health CNA, you won't be caring for dozens of residents in a facility. You'll have the opportunity to build one-on-one relationships with clients in their own homes, helping them remain comfortable, safe, and as independent as possible. This is a community-based role, so we're looking for CNAs who are comfortable traveling throughout Carbon County to provide care where our clients live. What You'll Do

As a Home Health CNA, you'll provide hands-on support that helps clients maintain their dignity, independence, and quality of life at home.

Your responsibilities

Providing compassionate personal care in clients' homes Assisting with bathing, grooming, dressing, toileting, and other activities of daily living Assisting with mobility, transfers, and positioning Helping with meals and hydration as appropriate Taking and documenting vital signs and other assigned observations Observing clients for changes in condition and communicating concerns to the appropriate member of the care team Providing companionship and emotional support Supporting clients in maintaining a safe and comfortable home environment Following individualized care plans and established care procedures Communicating professionally with clients, families, and members of the healthcare team Maintaining client privacy, dignity, and confidentiality Completing required documentation accurately and on time 🚗 Travel Throughout Carbon County Is Required This is a home health position, which means you'll travel to clients throughout Carbon County, Wyoming.

Applicants must be willing and able to travel throughout the Carbon County service area near Rawlins. We understand that traveling between clients is part of the job, which is why mileage is reimbursed at $0.60 per mile. Mileage reimbursement: $0.60/mile A reliable means of transportation and a valid driver's license are required.

Qualifications

Required Qualifications Minimum requirements include the following: Certified Nursing Assistant in good standing with the state (preferred, not required). High school diploma or equivalent. Current CPR/BLS certification Must have no disciplinary action in effect against certification. At least one year of experience as a certified nursing assistant in long-term care, home care and or hospice (preferred, not required).
