RN Home Care & Hospice Status: Full Time

  • Non-Exempt Reports to: Home Care/Hospice Manager __________________________________________________________________________________________ Purpose and Scope of Position This position is responsible for providing direct and indirect client care services primarily delivered on an intermittent basis primarily in the client’s place of residence. In the case of Hospice, care may be delivered in a long-term care facility or in an acute care facility. This position also directs Home Care Aides in performance of their duties. The primary responsibilities of the Homecare/Hospice RN are to coordinate the management of clinical activities of the patients during the assigned shift so that optimum quality of patient care is delivered at minimum cost. Each patient is assigned to an RN Case Manager who is responsible for implementation and updates of the plan of care. Nurses visiting the patient will give verbal or written reports on changes in the patient’s condition and need for changes in the plan of care. __________________________________________________________________________________________ Job Requirements Required Qualifications Graduate of an accredited school of nursing Current Montana RN Licensure, or eligible One year of nursing experience required CPR certification, or ability to obtain within 90 days of hire Current Montana driver’s license and the ability to be insured to operate motor vehicles Special Requirements Must be able to reason independently and work with minimal guidance. Must be able to interpret data and make sound judgments based on those interpretations. Must deal effectively with a wide variety of personalities and situations requiring tact, judgment, and poise. Must be able to adapt to quickly changing priorities and schedules. Must be able to maintain good working relationships with all co-workers and the general public and use good judgment in recognizing scope and authority. Must maintain professional conduct, patience and tact dealing with difficult situations with co-workers, patients, and visitors. Requires the ability to deal with patients with acute, chronic, and complex disease processes and those who are dying. Must be resourceful in dealing with simple to moderate complexity.
