- Friday 8:00 - 5:00 CHA is hiring a highly motivated, creative, and flexible individual to provide client support and case management across various locations including clinical, community, detention and harm reduction settings. Primary Responsibilities: Provide case management Conduct intakes and enrollment activities Follow care plans for clients Complete data tracking and monthly reports Document appropriately in electronic health record Accompany clients to meetings and participate in meetings of community groups to advocate client needs Education/Experience: Bachelor's degree in a human service field or similar degree with two years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness OR High school diploma or equivalent with 4 years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness and a peer support specialist certification OR equivalent education or experience. Lived experience preferred. Knowledge, Skills & Abilities: Knowledge of harm reduction principles, recovery and person first language Ability to plan and work collaboratively with community partners and CHA staff in person and virtually Ability to work with diverse groups of people and to deal tactfully with others Ability to exercise good judgement in appraising situations and making decisions Accurately record services rendered and to interpret and explain records Our Mission To improve individual and community health through services, education, and collaborative action.
