This role involves planning, organizing, and coordinating home health services, with a strong emphasis on community health and patient education.

Key Responsibilities

Develop and initiate individualized care plans in collaboration with patients and caregivers Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures Educate patients and caregivers to promote health and independence Identify and implement discharge planning needs Coordinate necessary equipment and services Supervise LVNs and HHAs Qualifications Active California RN license Minimum 1 year of direct patient care experience (home health preferred) Current CPR certification Valid driver’s license and insured, reliable vehicle The employer for this position is stated in the job posting.
