Licensed Independent Clinical Social Worker (LICSW) Full time
Job description
LICSW will serve as a team advisor, guiding the Care Plans, Assessments, and participants' needs within the Adult Day Health setting and their home environment. This individual will serve as a participant advocate, assist with all admission functions, and provide supervision support to the Nurse Case Manager. This individual will be responsible for the development, implementation, and evaluation of social service care plans for each assigned participant, coordinating services with other agencies/caregivers and continuously evaluating all service and care plans to see that needed services are received.
Experience in Dementia setting is highly preferred.
