Heritage Post Acute is seeking a compassionate, dedicated, and professional Licensed Master Social Worker (LMSW) to join our team and support the well-being of our residents.

We are looking for a qualified individual who is passionate about resident advocacy, care coordination, and helping residents and their families navigate the healthcare experience.

Responsibilities

• Provide social services support to residents and their families.
• Assist with admissions, discharges, and transition planning.
• Participate in interdisciplinary care plan meetings.
• Provide emotional support, counseling, and resource referrals as appropriate.
• Advocate for residents' rights, dignity, and quality of life.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Collaborate with nursing, administration, and other departments to ensure quality resident care.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current LMSW license in the applicable state.
• Strong communication, organizational, and interpersonal skills.
• Experience in long-term care, skilled nursing, or healthcare preferred.
• Ability to work collaboratively with residents, families, and the interdisciplinary team.
