Registered Nurse (RN) – Hospice Inpatient Unit | PRN Nights Ohio’s Hospice Location: Hospice Inpatient Unit at Licking Memorial Hospital Employment Type: PRN (As Needed) Shift: Nights, 6:30 p.m. – 7:00 a.m. Weekend Requirement: Every third weekend Position Overview Ohio’s Hospice is seeking a compassionate and skilled Registered Nurse (RN) to join our Hospice Inpatient Unit team at Licking Memorial Hospital.

As a member of our clinical team, you will help promote comfort, dignity, and quality of life for patients while providing support to their families throughout the hospice journey.

Key Responsibilities

Provide comprehensive nursing care through assessment, planning, implementation, and evaluation. Assess patients’ physical, emotional, psychosocial, and spiritual needs. Develop and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary hospice team. Evaluate patient conditions and adjust care plans, medications, interventions, and visit frequency as needed.

Recognize changes in patient status and promptly communicate significant findings to physicians and appropriate team members. Assist in determining the appropriate level of hospice care based on patient needs. Participate in hospice admissions, interdisciplinary team (IDT) meetings, and care conferences. Maintain accurate and timely documentation of assessments, physician orders, interventions, and clinical updates in the electronic health record.

Educate and support caregivers, Personal Care Specialists, volunteers, and other members of the care team. Provide clear shift-to-shift communication using SBAR and other approved handoff methods. Collaborate with interdisciplinary colleagues and other departments to promote coordinated patient care. Participate in quality improvement initiatives that support patient safety, care quality, and organizational goals.

Assist with the orientation and mentoring of new clinical team members as assigned.

Qualifications

Graduate of an accredited nursing program; Bachelor of Science in Nursing (BSN) preferred. Current, unrestricted Registered Nurse (RN) license in the State of Ohio. Two years of acute care nursing experience preferred. Certified Hospice and Palliative Nurse (CHPN) credential preferred. Ohio’s Hospice supports eligible employees interested in pursuing certification.

Current CPR certification. Strong computer skills and the ability to maintain accurate electronic medical records. Valid driver's license and a safe driving record. Automobile insurance meeting applicable state minimum requirements. Ability to travel safely when required, including in varying weather and driving conditions.
