• Newport, KY, Salary Range: $135000.00 - $142000.00Salary/year, Health. Point is in search of a full time Family Nurse Practitioner for our Northern Kentucky locations. Health. How to apply for loan forgiveness Base: $135,000-$142,000 (based on years of experience Annual Quality Bonus Potential: $23,000 Monthly production bonus: unlimited $10 for every visit over 263 per month Combined average annual salary:$160,000 Hours-40 hours per week, rotating schedule 3-5 days per week and 2 evening per week and 1-2 Saturdays per month.(some months will be 1 Saturday and some will be 2 Saturdays) Paid Time Off: 30 Days of PTO Holidays: Up to nine Paid Holidays and as well as your Birthday off paid. Years of Service Award: Continuous Years of Service bonus. Starting at year 2 with $4,500 with progressive increases. 401k Plan: $1 for $1 match on first 3% of income and $0.50 on the $1 for 4th and 5th% for a max match at 4% of your income. This is a Safe Harbor Plan so you are 100% vested on the company match immediately. Health/Dental/Vision: Organization pays a portion of all. Health options included PPO and HSA plan options. Group Term Life Insurance: Company paid $80,000 benefit; option for additional employee paid voluntary coverage Long Term Disability: Option available - employee paid Extended Illness Paid Time: Accrued extended illness time up to 8 weeks.
