How to apply for loan forgiveness Base: $110,000-$115,000 (based on years of experience Annual Quality Bonus Potential: $23,000 Monthly production bonus: unlimited $10 for every visit over 263 per month Combined average annual salary:$160,000 Hours-Mon: First pt appt time 8:30am | Last pt appt time 4:30 (Cov) Tues: First pt appt time 8:30am | Last pt appt time 5:30(Nwp) Wed: First pt appt time 8:30am | Last pt appt time 6:00(Cov) Thur: First pt appt time 8:30am | Last pt appt time 4:00(Cov) Fri: First pt appt time 8:30am | Last pt appt time 4:00(Cov) Half an hour for lunch on each of these days Note that the last appt time is the start of the appointment For coverage some Covington days may be changed to Newport from time to time Paid Time Off: 30 Days of PTO Holidays: Up to nine Paid Holidays and as well as your Birthday off paid. Years of Service Award: Continuous Years of Service bonus. Starting at year 2 with $4,000 with progressive increases. 401k Plan: $1 for $1 match on first 3% of income and $0.50 on the $1 for 4th and 5th% for a max match at 4% of your income. This is a Safe Harbor Plan so you are 100% vested on the company match immediately. Health/Dental/Vision: Organization pays a portion of all. Health options included PPO and HSA plan options.
