Community Health Worker

The Free Clinic of Culpeper is looking for a compassionate, community-minded Community Health Worker (CHW) to help bridge the gap between healthcare and the people who need it most! In this rewarding role, you'll support uninsured adults by helping them navigate healthcare services, overcome barriers to care, connect with valuable community resources, and take charge of their health.

From welcoming new patients and providing health education to coordinating care, assisting with referrals, and following up after hospital visits, you'll play an important role in ensuring patients receive the support they need. We're looking for someone who is approachable, organized, a strong communicator, and passionate about making a difference.

The ideal candidate enjoys building meaningful relationships, working with diverse populations, and helping individuals overcome challenges related to healthcare access, transportation, housing, and other essential needs. Bilingual skills in English and Spanish are a plus! A high school diploma or equivalent is required, along with CPR/AED certification or the ability to obtain it within 90 days.

Experience in healthcare, social services, community outreach, or a related field is preferred. Strong computer and organizational skills are also important.
