Resident Care Coordinator

Job Details: Job Location: OAKS AT OAKLAND PLANTATION

• LEESBURG, GA, Position Type: Full Time, Salary Range: Undisclosed, PRIMARY PURPOSE To assist the Wellness Director with maintaining Resident Medication and Medical Records according to policy and State Regulations. To assist Executive Director with Administrative Duties.

RESPONSIBILITIES

1. Assist Wellness Director with checking in new cycles of medications every 14 days
2. Maintain Med Carts (PRN’s, Creams) every 2 weeks
3. Daily check of Medication reorder sheet
4. Ordering Oxygen & Oxygen supplies
5. Weekly MAR Audits (missing initials, signature) and communicate to Wellness Director
6. Executing Medication Changes for Residents from Dr.’s appointments a) Fax order to Pharmacy b) Make copy of order and give to Wellness Director c) Check new order and write change in MAR when delivered
7. Send copy of MAR & Medical Appointment form with resident when going to Dr. appointment ADMINISTRATIVE:
8. Assist at Front Desk greeting guests/residents, vendors & deliveries
9. Assist answering Phone Calls
10. Conducting Orientation for New Hires
11. Maintaining Continuing Education Unit (CEU’S) Records & scheduling CEU classes 1X monthly
12. Track CPR/First Aid Certification
13. Assist E.D. with auditing Employee Files
14. Care Staff Scheduling and any shift changes
15. Replenishing Copies of Forms (Shower/Laundry/Pager Logs)
16. Take On-Call 1 weekend per month
17. Assist with Tours when needed Qualifications: QUALIFICATIONS
18. Must be at least 18 years of age
19. Must have received High School Diploma
20. Must have a satisfactory criminal record check
21. Must have a physical exam by a licensed physician
22. Must have a Negative Drug Screen
23. Must have C.P.R. & First Aid Certification
24. Must dress in Professional attire
25. Must be able to communicate effectively to convey information to residents, families, supervisor, Wellness Director and Executive Director
26. Must be able to lift 50 lbs. As well as complete tasks which involve walking, standing reaching, pushing, and grasping
27. Must understand risk of exposure to infectious waste, disease, chemicals, solutions, and disinfectants
28. Must be able to react in emergency situations
29. Must have a valid Certified Medication Licensure.
30. Must have experience in supervising and scheduling.
