Field RN (Home Health)

AHH West Central is hiring a FT RN field nurse in the Natchitoches, Many, Noble, Zwolle, LA and surrounding areas. The RN field nurse is responsible to manage patient health problems and needs, develop and implement a plan of care, team schedules, and maintain medical records. The RNFN shall utilize the team to administer and carry out Plan of Care, advise the team on health maintenance and disease prevention and provide case management.

RNFN is also responsible for assessing patient health problems and needs. In addition, administers nursing care to ill, injured, convalescent, or disabled patients, and may advise patients on health maintenance and disease prevention or provide case management.
