Assess and manage pain and symptoms. Develop and coordinate individualized care plans. Educate and support patients and families. Collaborate with the interdisciplinary hospice team. Document hospice eligibility and supervise hospice aides.

QUALIFICATIONS

Active Pennsylvania RN license in good standing. 1 year of hospice nursing experience preferred. Strong clinical, communication, and organizational skills. Compassionate, dependable, and team-oriented. Valid driver's license, reliable vehicle, and auto insurance. Ability to travel within assigned territory. Ability to perform essential physical job duties, including lifting and assisting with patient transfers.
