Responsibilities

• Provide skilled nursing care to patients in their homes, including medication administration, wound care, and monitoring vital signs.

• Document patient care activities accurately and maintain compliance with company and regulatory standards.
• Educate patients and their families on health maintenance and disease prevention.
• Respond promptly and effectively to changes in patient condition or emergencies.
• Maintain professional standards and confidentiality at all times.

Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Experience in home health care or a similar nursing environment preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Reliable transportation to travel to patient homes.
