Care Manager Trainer

As the Care Manager Trainer, you will lead the development, implementation, and continuous improvement of the tailored care management training program. You will prepare team members to deliver consistent, person-centered care through onboarding, role-specific orientation, core competencies, contract and regulatory training, continuing education, and competency validation.

Working with program leaders, you will align curriculum with care management tools, workflows, documentation standards, and workforce performance needs. The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living.

Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
