LPN - NP Terrace - Med Pass

Beloit Health System is looking to add Per Diem Med Pass LPNs to ou r NorthPointe Terrace Team!!

The LPN**- Med Pass** administers medications to residents who are functionally, physically, or socially impaired and who need assistance with supportive, nursing, and personal care. All responsibilities will be conducted in a manner that is consistent with the philosophy of Beloit Memorial Hospital's and NorthPointe Terrace's mission, vision, and values.

• Assesses patient for change of condition from report, care notes and fall reports

• Ensures that new orders are transcribed, ordered and received and discusses changes with resident

• Reviews labs, orders and medications with monitoring of administration and completion of documentation in relation to each review

• Participates as a resource and team member for the PCAs during medication passes.

• Used base of knowledge to educate staff, residents and families

• Writes care notes on any changes in resident condition and follows up on previous notes

Job Requirements

• Must be licensed as a LPN in Illinois.

• Must be able to think, act, and intervene independently in both routine and emergency situations.

• Must be self-motivated and resident centered.

• Must be able to relate to all residents and staff in a courteous, professional, and compassionate manner under all circumstances.

• Must be able to work flexible hours and participate in holiday activities with residents.

• Valid driver's license required.

• CPR certification within 90 days of hire

• Maintains appropriate levels of confidentiality at all times.

• Works collaboratively with peers and team members.

• Follow up promptly and thoroughly with appropriate staff, residents, or other individuals regarding complaints, problems, and concerns.

• Adheres to established hospital and NorthPointe Terrace policies, rules, and regulations.
