ACT Team Specialist

As a member of an Assertive Community Treatment (ACT) team, functions as a case manager with primary responsibility for providing assertive treatment services, implementing plans for treatment, rehabilitation and support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs.

Acts as a member advocate, coordinates services and discharge plans, and assists in crisis intervention. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.
