POSITION SUMMARY
The Clinical Care Manager provides clinical and case management services for adults ages 18+ years of age. The Clinical Care Manager will foster positive working relationships with clients, families and outside key stakeholders and utilize a strength based-solution focused model of care. SALARY $58,000 EDUCATION AND EXPERIENCE/QUALIFICATIONS Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required.
Must have valid driver's license and access to reliable transportation. Must be willing and able to drive the mobile medical unit. Licensure Requirements for Clinical Positions Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut.

Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant. Demonstrates knowledge of psychopathology and other behavioral health concerns and primary modalities of psychotherapeutic intervention.
Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to model and program expectations. Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.
Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings. Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.
Documents all client related information in compliance with agency policy. Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives. Provides services in the office, on the mobile medical unit, and in correctional facilities.
Responsible for safely driving and operating the Mobile Medical Unit, including all associated operations and maintenance tasks. Tasks may include but are not limited to operating and/or maintaining the unit’s slide, leveling system, propane, gas, waste and water systems. Oversee that all necessary supplies and equipment are organized and loaded on and off the mobile medical unit to ensure it is stocked for events.
Supplies and equipment may include scales, vitals cart, AED, point of care testing, IT equipment, promotional items, and paperwork for registration. Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
