You'll: Complete Start of Care (SOC) assessments and OASIS documentation Evaluate patients and develop individualized plans of care Coordinate care with physicians and the interdisciplinary team Ensure patients receive a smooth and successful transition to home health services Provide education and support to patients and their families Help patients achieve their healthcare goals and remain safely at home WHAT WE'RE LOOKING FOR Current, active Pennsylvania RN License Previous Home Health experience REQUIRED Strong OASIS and Start of Care assessment experience Excellent clinical judgment and assessment skills Independent, dependable, and organized Valid driver's license and reliable transportation DON'T WAIT — WE'RE INTERVIEWING NOW!
