Nurse Practitioner Palliative Medicine (Outpatient) - Atlantic County

Seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine outpatient team. This NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within outpatient department, with a particular focus in Atlantic County. This includes settings such as the patient’s home, care facilities and clinics.

This is a full-time position, where the Nurse Practitioner will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care. This role requires skill in establishing and maintaining relationships with new partners.
