Per Diem Nurse Practitioner - Palliative Medicine (Outpatient)

Seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine Outpatient team. The NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within outpatient department. This includes settings such as the patient’s home, care facilities and clinics.

This is a per diem position, where the Nurse Practitioner will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care. The NP will develop and maintain internal and external relationships, while meeting the needs of our patients and their families.
