Personal Supports Case Manager (FT)

• Fluency in American Sign Language. Job Description: COMMUNITY SUPPORT SERVICES For the DEAF, Inc. Fluent in American Sign Language. POSITION: Personal Supports. STATUS: Full-Time (40 hours) OR Part-Time WHO IS A CASE MANAGER: A Direct Support Professional who assists Deaf or Deaf-Blind Adults with Intellectual/Developmental Disabilities. Assistance includes Home-based assistance with personal care, cleaning, laundry, cooking, medication management, and assistance with mail. Community-based assistance with medical appointments, shopping, socialization, and activities/events. Other duties as identified in the annual Person-Centered Plan (PCP) meeting. EXPECTATIONS: Excellent time management skills, well organized, team player, positive attitude, patient and respectful. Annual physical and flu vaccinations are required.

QUALIFICATIONS

Bachelor’s degree or High School Diploma with 3 years relevant experience. Valid MD Driver’s License with 2 points or less. Fluent in American Sign Language (ASL). APPLICATION DEADLINE: Until filled.
