Connect clients and families with appropriate treatment, stabilization services, and community resources. Clinical Services & Follow-Up Complete client-centered assessments and accurately document services in the electronic health record. Develop individualized plans focused on risk mitigation, connection to services, and ongoing support.
Provide short-term follow-up services following mobile response, which may include: Psychotherapy, Case management, Advocacy, Referrals, and resource coordination Follow up with clients and make referrals to stabilization services when appropriate. Collaboration & Care Coordination Consult with internal and external providers regarding mobile response access, risk mitigation, care triage, and other clinical or operational needs.
Collaborate with clients, referral sources, hospital staff, police departments, and other community stakeholders. Coordinate services and communicate effectively across providers and systems. Participate in team problem-solving to improve service delivery and program efficiency. Documentation & Quality Complete clinical documentation within required timeframes.
Follow MRSS workflows and data collection requirements. Maintain documentation in accordance with agency standards. Continue advancing clinical knowledge and training in required evidence-based interventions and fidelity model practices. Follow all agency policies and procedures.
