What You'll Do

Provide skilled home health Physical Therapy care: Directly help adult and geriatric homebound patients achieve better health and independence in their own homes. Assess and plan: Evaluate patient needs and create personalized care plans. Manage your patient visits: Set your own schedule and caseload. Document efficiently: Use our simple EMR system for all notes.

Collaborate: Work with a supportive care team, ensuring coordinated care amongst supervising therapists, other disciplines, and within our administrative team to optimize patient outcomes.
