Licensed Practical Nurse (LPN)

We're looking for a compassionate Licensed Practical Nurse to be the steady, trusted connection between our patients, their families, community staff, and our Nurse Practitioners across Ada & Canyon County.

What You'll Do

Serve as the go-to clinical contact for the assisted living and memory care communities in your county Partner with our RNs and Nurse Practitioners to schedule and prioritize patient visits Process referrals, medication requests, and refills as directed by providers Coordinate labs, durable medical equipment (DME), and other clinical resources Send, track, and follow up on provider orders Educate patients, families, and caregivers so everyone feels confident in the care plan Keep accurate, timely documentation in our electronic medical record What You Bring Active LPN license in Idaho 1+ year of nursing experience preferred Experience in geriatrics, assisted living, memory care, palliative care, or hospice is a plus Current CPR/BLS certification Strong organization and clear communication, both verbal and written Valid driver's license, auto insurance, and reliable transportation for local travel Why Auburn Crest Pay: $70,000 to $75,000 Benefits: health, PTO, retirement, etc.
