We are seeking an experienced and compassionate Director of Social Services to join our leadership team. This full-time leadership position plays a vital role in enhancing the quality of life for residents by overseeing social services, resident advocacy, care coordination, and discharge planning within our long-term care community. $40.00-45.00 hourly rate plus $5,000 Sign on Bonus! The ideal candidate is an experienced long-term care professional who is passionate about resident-centered care, skilled in navigating complex healthcare needs, and committed to supporting residents and families throughout their journey.

What You'll Do

Lead and oversee all social services operations within the facility. Champion resident rights, dignity, independence, and quality of life. Coordinate discharge planning and care transitions to promote positive outcomes. Partner with interdisciplinary care teams to ensure comprehensive, person-centered care. Conduct psychosocial assessments and develop individualized service plans.

Provide support, counseling, education, and guidance to residents and family members. Connect residents and families with community resources and support services. Facilitate care plan meetings, family conferences, and resident advocacy initiatives. Ensure compliance with federal, state, and professional standards. Maintain accurate and timely documentation and records.

Contribute to quality improvement efforts and organizational excellence.

Qualifications

What You'll Bring

Required Qualifications Previous experience serving as a Director of Social Services in a Long-Term Care setting Education: Master's-level social work preparation (MSW-prepared) In-depth knowledge of long-term care regulations, resident rights, care planning, and discharge processes. Strong leadership, communication, and relationship-building skills.
