Position located in Sheboygan County. $5000 Sign on Bonus. Case Manager Job Responsibilities: Manages an assigned caseload of program members in collaboration with an RN Case Manager Completes comprehensive member assessments, member centered care plans, & functional screens Provides ongoing care coordination and quality assurance monitoring of services Maintains required documentation in adherence to established standards.
Case Manager Job Requirements:
- Education: Bachelor’s Degree in a Human Services or closely related a rea required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
- Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred .
- Certification: Eligibility for long-term care functional screener certification is required
- Travel : Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
