As a Physical Therapist, you will help bring that mission to life by helping residents move better, feel stronger, and reach the highest level of function possible. What You’ll Do As a Physical Therapist , you will evaluate residents, develop individualized plans of care, and provide skilled therapy services designed to improve mobility, safety, and independence.

You will: Complete comprehensive physical therapy evaluations. Develop individualized treatment plans based on resident needs, goals, and functional abilities. Provide skilled therapy focused on strength, balance, mobility, endurance, coordination, and functional movement. Help residents improve their ability to safely complete transfers, walking, and other daily activities.

Monitor progress and adjust treatment plans as needed. Establish measurable therapy goals and track progress toward those goals. Educate residents and families on exercises, mobility strategies, safety, and continued progress. Provide recommendations for assistive devices and adaptive equipment as appropriate. Complete documentation accurately and on time.

Collaborate closely with nursing, providers, therapy team members, and other disciplines. Participate in care planning and interdisciplinary communication. Support safe discharge planning and transitions when appropriate. Maintain therapy equipment and treatment areas in a clean and safe manner. Follow established clinical, infection-control, safety, and regulatory requirements.
