Oak. Med Home Nursing & Rehabilitation is growing our network of Physical Therapist Assistants throughout West Michigan!

We are looking to connect with licensed PTAs interested in providing one-on-one, home-based care to automobile accident and workers’ compensation patients. Whether you’re currently working in a clinic, looking to supplement your income, or interested in building a flexible home-based caseload, we’d love to hear from you.

Carry out assigned treatment activities consistent with the established plan of care. Monitor patient response to treatment and communicate changes or concerns to the supervising Physical Therapist. Document visits and patient progress accurately and in a timely manner. Communicate professionally with patients, families, and members of the healthcare team.

Follow applicable professional standards, supervision requirements, and organizational policies.

Qualifications

Current, unrestricted Physical Therapist Assistant license in the State of Michigan. Graduate of an accredited Physical Therapist Assistant program. Reliable transportation and ability to travel to assigned patient homes. Strong communication, organization, and documentation skills. Ability to work independently within the PTA scope of practice and applicable supervision requirements.
