Position Summary

We are seeking a compassionate and experienced Licensed Social Worker (LSW) for our social services department. This role is responsible for overseeing the psychosocial well-being of residents, coordinating discharge planning, and serving as a vital liaison between residents, families, and the interdisciplinary care team.

Key Responsibilities

Assess residents’ psychosocial needs and develop individualized care plans Provide emotional support and counseling to residents and families Coordinate discharge planning and community resource referrals Advocate for residents’ rights and ensure their needs are met Maintain accurate documentation in compliance with state and federal regulations Collaborate with nursing, therapy, and administrative teams to support holistic care Supervise and support social services staff, if applicable Qualifications: Current and valid Licensed Social Worker (LSW) license in the state of Ohio Bachelor’s degree in Social Work Minimum of 2 years of experience in a healthcare or long-term care setting Strong communication, organizational, and problem-solving skills Knowledge of Medicare/Medicaid regulations and discharge planning Commitment to resident-centered care and ethical practice Benefits: 401(k) with employer match Medical, dental, vision, and life insurance Paid time off (PTO) and holiday pay Competitive pay Supportive and mission-driven work environment #FSHP
