We’re looking for an RN Case Manager who thrives on connection, critical thinking, and helping patients live their best possible lives to provide care across the greater Flint area (Lapeer/Davison/Fenton/Swartz Creek, etc.) In this role, you will: Conduct thorough nursing assessments and create individualized care plans. Deliver skilled nursing interventions in the home. Teach patients and families how to manage health conditions and medications. Coordinate with physicians, therapists, and other team members to ensure seamless care. Keep documentation accurate, timely, and compliant with state, federal, CHAP, and Joint Commission regulations. Hold a current Michigan RN license. Have at least two years of nursing experience (home health preferred). Are compassionate, resourceful, and an excellent communicator. Have a valid driver’s license, insurance, and reliable transportation.
