Clinical Care Specialist
Job description
Horizon is looking for a Clinical Care Specialist, based in East Amherst, NY. The Clinical Care Specialist will work with children, adolescents, and families to provide referral, linkage, and care coordination to appropriate services. This may include coordination with schools, pediatricians, inpatient mental health providers, inpatient substance use treatment, day treatment, and health home providers.
This position is full-time, Monday-Friday with at least one evening shift.
