We are seeking a compassionate, knowledgeable, and dependable Registered Nurse (RN) to join our team and provide nursing oversight and clinical support to individuals receiving services through our OPWDD-certified programs. The Registered Nurse is responsible for providing and coordinating nursing services to individuals supported by CMA.

The RN promotes the health, safety, and well-being of the individuals we serve while ensuring compliance with applicable OPWDD regulations, and CMA policies and procedures. The RN works collaboratively with individuals, families, healthcare providers, Care Managers, direct support professionals, and program leadership to ensure that healthcare needs are identified, addressed, documented, and appropriately followed up.

Essential Responsibilities Provide nursing assessments, monitoring, and clinical oversight for individuals receiving services. Develop, implement, and evaluate individualized nursing care plans. Monitor individuals for changes in health status and take appropriate action when concerns arise. Administer medications and treatments in accordance with physician orders and agency policies.

Review medication administration records and follow up on medication errors, missed doses, discrepancies, and other concerns. Coordinate medical appointments, follow-up care, hospitalizations, and transitions of care. Communicate with physicians, pharmacies, hospitals, families, guardians, and other healthcare professionals as appropriate.

Provide health education and promote preventive healthcare and healthy lifestyle practices. Provide training, guidance, and clinical support to Direct Support Professionals and other agency staff regarding individuals' healthcare needs. Ensure staff follow appropriate health, safety, infection control, medication administration, and emergency procedures.

Contribute to the development and review of life plans and related documentation. Maintain accurate, timely, and confidential nursing and medical records. Monitor compliance with required medical appointments, health assessments, treatments, and follow-up recommendations. Identify health and safety risks and recommend appropriate interventions.

Participate in quality assurance activities, audits, investigations, and corrective action plans as needed. Ensure nursing practices are consistent with OPWDD regulations and CMA policies and procedures. Respond appropriately to medical emergencies and provide necessary nursing interventions. Perform other duties and responsibilities as assigned.

Qualifications

Current and valid New York State Registered Professional Nurse (RN) license. Bachelor's degree in Nursing. Previous experience working with individuals with developmental disabilities is preferred. Experience in a community-based, residential, healthcare, or human services setting preferred. Knowledge of OPWDD regulations and requirements preferred.

Strong clinical assessment, organizational, communication, and documentation skills. Ability to work collaboratively with individuals, families, healthcare providers, and interdisciplinary teams. Ability to exercise sound clinical judgment and respond effectively to changing medical needs. Valid driver's license and reliable transportation may be required depending on program assignment.

Work Sites Serving Nassau and Suffolk Counties, NY Position Option 1 Coverage locations: Glen Cove, Kings Point, Westbury, and Dix Hills, NY Position Option 2 Coverage locations: Malverne, Massapequa, Merrick, and Westbury, NY Work Hours / Schedule Monday–Friday with flexible scheduling options This position includes on-call duties for telephone triage coverage.
