• Carbondale, PA, Salary Range: $19.50 - $22.90Hourly, $2,000 Sign on Bonus Blended Case Manager (BCM), will promote resiliency and recovery for children, adolescents, adults and their families by accessing, coordination, mentoring, and advocating for community resources and services.Qualifications: Bachelor’s Degree in Human Services or related field.
