Hospice LCSW
Job description
Provides social work services following all applicable agency policies and procedures. The Medical Social Worker reports to the Hospice Director. Social work is a core hospice service available when necessary to meet patient need. The Social Worker is a member of the Hospice Interdisciplinary Group (IDG). IDG members are available to attend patient deaths 24 hours/day, 7 days a week.
