Supplemental Service Application

This application is intended for individuals who have been pre-identified or referred for specialized supplemental roles within the district. This includes: Athletic Event Staff Athletic Trainers Community Education Program Instructors or Lifeguards Extra-Curricular Club Advisors & Helpers Athletic Coaches Note: This posting is used for administrative onboarding and compliance.

If you have been directed to this page by the Athletic Director or HR Representative, please complete the application to initiate your background checks and payroll processing. If you have not, you will not be contacted. Minimum Requirements: You must have all of the required documentation to complete the application process.

Clearance Information Act 151 Childline Act 34 PA State Police Act 114 FBI Fingerprinting TB Test Results (must be dated within 3 months of submission date) VALID Government Issued ID (legible copy) Must have prior authorization or referral from the relevant Department Supervisor.
