Job Title: Physician (Internal Medicine / Family Medicine) Location: Lake Wales, FL (On-site) Job Type: Full-Time Job Summary: A premier post-acute medical group is seeking an autonomous Post-Acute Care Primary Care Physician (MD/DO) to lead facility-based care in Lake Wales, FL. This role is ideal for a clinician who enjoys the pace of transitional care, performing daily rounds across Skilled Nursing Facilities (SNFs).

You will take the lead on managing high-acuity patients, welcoming new admissions, and providing structure and supervision to local Advanced Practice Providers (APPs). Schedule: Full-time: 5 days per week Hours: Monday to Friday, 8:00 AM – 5:00 PM Key Responsibilities: Perform daily rounds at designated Skilled Nursing Facilities (SNFs) in the Lake Wales area.

Review, examine, and admit incoming new patients to the sub-acute units. Directly intervene and manage care for higher-risk or clinically declining patients. Supervise, guide, and sign off on charts for local APPs.

Qualifications

& Requirements: Board Certified (BC) or Board Eligible in Internal Medicine or Family Medicine (Board Certification is NOT a requirement). Active Florida Medical License. ACN (Area of Critical Need) license holders are accepted and welcome to apply. Strong clinical decision-making skills in a post-acute setting.

Compensation
