Case Manager - Streetwork Lower East Side

Streetwork Project is an innovative outreach-counseling program for homeless youth in Manhattan, which utilizes a harm reduction approach. Under this umbrella, Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex).

The Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short and long term care necessary to meet the wide range of needs related to homeless adolescents.
