In This Role, You Will Conduct comprehensive nursing assessments Develop individualized plans of care Deliver skilled nursing interventions in the home Educate patients and families on disease management and medications Coordinate with physicians, therapists, aides, and interdisciplinary team members Complete accurate and timely documentation in Well.
