Physical Therapist/ Home Care Case Manager South Dayton, Miamisburg, Germantown, Middletown, Dayton,. Englewood, Vandalia, Springfield, Urbana, New Carlisle and Yellowsprings General Summary/Responsibilities: The Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences.

When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services. The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.

The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

FIDRNOCT Qualifications Education Minimum Level of Education Required: Bachelor's degree Additional requirements:  Type of degree: Bachelors

  • Graduate of an accredited Physical Therapy Program.  Area of study or major: Physical Therapy Licensure/Certification/Registration  Current licensure to practice Physical Therapy in Ohio.  CPR Certification Experience Minimum Level of Experience Required: 1 - 3 years of job-related experience Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care. Prior specific functional responsibilities: N/A Preferred experience: One year clinical experience in acute care setting preferred Other experience requirements: N/A Knowledge/Skills  Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.  Must be able to creatively adapt multiple environments and situations for successful outcomes.  Must be self-directed and able to work with little supervision.  Computer skills preferred.  Home care experience preferred.
