What You’ll Do ▶ Provide comprehensive primary care for age-specific patients in Family Medicine and Pediatrics ▶ Diagnose and treat acute and chronic health conditions within your scope of practice ▶ Provide health education, prevention strategies, and health-management guidance ▶ Collaborate with physicians, clinicians, and Care Team members to coordinate patient care ▶ Document patient encounters accurately and timely in the EHR ▶ Support quality improvement, quality assurance, and department goals ▶ Participate in after-hours on-call coverage and provide coverage between PCHG locations when needed What’s In It for You ▶ Mission-driven work that directly improves community health ▶ Collaborative physician and Care Team environment ▶ Opportunities to mentor and support developing providers and clinical staff ▶ Quality-focused practice with an emphasis on continuous improvement ▶ Professional growth within a community health organization ▶ The opportunity to build lasting relationships with patients and families Qualifications ▶ Master ’ s degree from an accredited Nurse Practitioner program with a Family Medicine or Pediatrics clinical specialty ▶ Current Florida APRN license ▶ Current BLS certification ▶ Current DEA license strongly preferred, or ability to obtain ▶ Three years of Nurse Practitioner experience preferred ▶ Ability to obtain and maintain Epic certification and annual compliance training Working Conditions & Physical Requirements

• Ability to lift 20 lbs. regularly and 30–50 lbs. occasionally
• Ability to sit for extended periods
• Direct exposure to computer screens
• May be exposed to contagious/infectious diseases
