Lutheran Social Services of WI & Upper MI is seeking a full-time Care Coordinator for Milwaukee County's Children's Long Term Support (CLTS) Waiver Program. Coordinators help families access services for children with developmental, physical, or severe emotional disabilities, ensuring they can thrive safely at home and in the community.
Responsibilities include
assessment, service planning, coordination, and documentation. Candidates must hold a BA/BS in a human services field with 1 + year of experience.
