Medical Assistant - Street Medicine

Join our Street Medicine team in a unique dual-role as an Enhanced Care Management (ECM) Community Health Worker and Medical Assistant. In this impactful position, you'll connect individuals experiencing homelessness with vital health and community resources while supporting field-based patient care alongside a Physician Assistant. As a key member of our multidisciplinary team, you'll help improve health outcomes through outreach, engagement, care coordination, and compassionate patient support. This dynamic role also includes driving a Neighborhood vehicle to support Street Medicine services in the community.

Schedule: Monday-Friday from 8:00 am-5:00 pm.
