Job description
Lutheran Social Services of Wisconsin and Upper MI is currently seeking a Service Facilitator to join our Comprehensive Community Services (CCS) team serving Trempealeau County. CCS is a team-based wraparound program for individuals with mental health needs and/or substance use concerns. The Service Facilitator provides community-based services to adults and children with high level needs across environments; including home, school, and community.
This is a full-time, benefit eligible position.
