Pace Geriatric Services Specialist (Central Valley Pace - Modesto)

Responsible for providing support and advocacy services to patients of the Central Valley PACE program, including participant screening, assessment, planning, coordination of patient services and care, counseling and referral.

Schedule: Monday – Friday, working 8:00am – 5:00pm. Location: Central Valley PACE Center in Modesto, Ca.

This position will be based and trained at our Modesto center.

Compensation

$35.25 – $37.01 an hour
