Children's Care Coordinator - Brewer

Morrison Center is seeking a Children’s Care Coordinators , to join our dynamic, multidisciplinary team in Brewer, Maine Care Coordinators assist individuals and their families to identify strategies that will enable them to meet their goals.

Documentation in accordance with regulatory agencies Case Manager must represent Morrison Center in the community and act as a liaison with other agencies. Perform monthly home visits, school observation Support families at IEP meetings, team meetings and medical appointments Complete 90 day continued stay reviews with the family Morrison Center is a private, non-profit organization whose mission is to provide comprehensive, individualized support services to children and adults with developmental disabilities.
