Mental Health Case Manager (Community Navigator)

Please Note:  Ottawa County Community Mental Health is currently transitioning to an independent authority. As of January 1, 2027, this position will be a part of the Ottawa County Community Mental Health Authority.  Under the supervision of the Director of Access and Connections, screens service requests for Community Mental Health services, connects individuals to resources, supports, and services within the community, provides referrals and assistance with navigating the behavioral health system, and coordinates care with community agencies.
