Intensive Community Care Specialist

Provides care and services for adults with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and client’s living environment.  Services may include skill building, modeling, coaching, coordination and connection to resources and natural supports.

This position works within a team to navigate and meet the comprehensive needs of the client.
