Specialized Case Manager - San Diego County

Pay Range

$28.00 - 30.00

The Specialized Case Manager maintains the foundation of Catalyst case management while incorporating participant-focused Housing Access Services responsibilities. This position manages a mixed caseload that may include Housing Access Services participants as well as participants from other Catalyst programs, including ILS, SLS, Tailored Day, and other assigned service areas. The role focuses on housing navigation, housing stability planning, tenancy support, service coordination, and person-centered advocacy to help participants access and maintain safe, stable, and affordable housing.
